- Passport Size Photo
- Self Attested Declaration Form

Application Fee:
- Fee Payment By Internet Banking: ₹ 5 + ₹10 = ₹ 15/-
- Fee Payment By Using Debit Card Credit Card ATM, fee will be applicable: ₹ 18 + 10 = ₹ 28/-

How to fill online form, and Important instructions.
Step-1st: Registration to create your User ID and Password
Step-2nd: Login with using your User ID and Password
Step-3rd: Fill your Certificate Form carefully
Step-4th: Upload Your scaned Documents
- Photo (Photo size must be under 50 kb)
- Self Declaration Form (attested by himself)
- Upload your Identy card like Voter ID or RSAN CARD
- Other Documents
Step-5th: Deposit your Service Fee (Rs 15 to 28)
Step-6th: Print out your acknowledgement Slip
- Do not use any other person’s bank account for online exam fee payment. Sometimes during the online fee payment interruption. Your transaction may get interrupted, money is deducted from the account, but the payment institution is unable to make the payment, in which case you may not get the correct information and you may have to suffer loss.
- Finally Submit your form.
